You will need the driver's last name, the date of the accident, and the accident number.
The accident number should be entered as the four-digit year and eight numbers for the rest of the case number. Example 2024-00001234. (add zeros to get the eight digits)
It can take up to ten (10) business days for your report to be uploaded to CrashDocs.
